Who
can I include as a dependent on my dental plan?
Any of
the following:
A spouse
or common law partner.
A dependent
child to age 19, or 23 if full time student.
A disabled
child who is fully dependent on you for support.
What
if I want to add dependents to the dental coverage later?
You need
to include dependents now. You may add/drop dependent dental
coverage later if you have a change in family status due to
marriage, divorce, birth of a child, responsibility for a
child because of divorce, adoption, etc…
How
many times per year can I have my teeth cleaned?
Unlimited.
When
will I be able to use the plan once I sign up?
All coverage
starts on the first of the month. When we receive an application
either over the Internet, via fax or mail, your coverage will
be effective on the 1st of the following month. Applications
sent by mail must arrive to us no later than the 20th of the
month to be effective the first of the following month..
Example: We receive an internet application on 3-28-2004,
effective date will be 4-1-2004.
When
will I get a card?
If you
signed up over the Internet, you will get a confirmation e-mailed
to you. Your card is mailed to your home address in about
10-20 business days.
