  For family coverage, is there a separate deductible for each individual as well?
  Yes, each family member has a separate deductible.  However, the family deductible is two times the individual deductible - so if two or more family members are covered, only two have to meet the separate individual deductible to satisfy the family deductible.
  For Autograph HSA-Qualified plans, the family deductible applies to all covered family members, so one person's covered expenses could satisfy the deductible for the entire family.  For non-HSA Autograph plans, each family member has a separate deductible, but only two have to meet the separate individual deductible to satisfy the family deductible.
  Yes, each family member has a separate deductible.  However, the family deductible is two times the individual deductible - so if four or more family members are covered, only two have to meet the separate individual deductible to satisfy the family deductible.

Coverage Synopsis

The HumanaOne Portrait plan has a low $1000 or $2500 annual deductible for in-network coverage.  Once you've met your annual deductible, the plan pays 80 percent for most covered in-network medical services and you pay 20 percent.  All in-network office visits for covered illnesses and injuries are paid at 100 percent after your copayment.  Prescriptions are covered with copayments as low as $15 for common prescriptions.  Certain drug levels require meeting a separate prescription deductible.  This is the most comprehensive Humana plan.

The HumanaOne HSA Autograph plans let you choose from a variety of deductibles ranging from $1,500 to $10,000 after which the plan pays 100%, 80%, or 70% up to the coinsurance limit.  Once you reach your deductible and coinsurance limit the plan will pay 100% of covered expenses up to the lifetime maximum with no annual maximum.  You can choose whether or not to include prescription drug coverage.  Office visits are covered subject to deductible and coinsurance.  Routine preventative care is covered subject to coinsurance up to $300 per person annually.  These HSA-qualified plans are the most popular Humana plans, and can be coupled with a Health Savings Account.

The HumanaOne Non-HSA Autograph plans are similar to those above, but do not qualify to work with an HSA.  These plans have a variety of deductibles ranging from $2,500 to $10,000 after which the plan pays 80% or 70% up to the coinsurance limit.  Once you reach your deductible and coinsurance limit the plan will pay 100% of covered expenses up to the lifetime maximum with no annual maximum.  A $35 copay for up to 6 visits annually is available.  Routine preventative care is covered subject to coinsurance up to $300 per person annually.  Prescriptions are covered with copayments as low as $15 for common prescriptions.  Certain drug levels require meeting a separate prescription deductible.

The HumanaOne Monogram plan has a $7,500 deductible for individuals and a $15,000 deductible for families.  This is the most economical plan Humana offers.  Once you've met your annual deductible the plan pays 100% of covered expenses.  Prescriptions are covered with copayments as low as $15 for common prescriptions.  Certain drug levels require meeting a separate $1,000 prescription deductible.  Most routine preventative care is covered before deductible with labs and x-rays covered after deductible.
