How
the Plan Works
You
choose a deductible of $250, $500, $1000, or $2500. After
the deductible, the plan pays 80% of the next $10,000 in covered
expenses, and 100% of the remaining covered expenses up to $100,000
for each illness or injury up to a $1,000,000 lifetime maximum.
This plan does include office visits, but does not cover
prescription drugs. A complete description of the coverage

your coverage with you. The Student Health Insurance
we market is available to any student (up to age 29) either
living in or going to school in Colorado. Once you've
been accepted the coverage is yours to keep even when you
are no longer a student.

health insurance plans only cover you until you graduate
or very shortly thereafter, this plan can be renewed for
as long as you continue to pay the premiums.
Eligibility
Requirements
Any
undergraduate taking nine or more college credits, or any full-time
graduate student (as defined by the school), up to age 29, is
eligible to apply.
Preauthorization
If
you are insured under this plan, you must obtain authorization
for all hospital admissions, outpatient surgeries, and certain
other services. The number to call for preauthorization
is 1-800-454-5105. This number will also be on your student
health insurance identification card and the student health
insurance contract which you will receive.
No
HMOs or PPOs
With
this student health insurance plan you can visit the doctor
or hospital of your choice. No referrals are needed, and
no non-network penalties are incurred.
Money-back
Guarantee
If
you are not 100% satisfied with the student health insurance
plan, you can return the contract within 10 days of delivery
for a refund. If a cancellation request is received after
the 10-day free look, a prorated refund will be provided as
described in the contract.
