How
to Apply
Please
note that all applications will be submitted in the name of Wiley
Long, the president of ColoHealth.
If you have already been working directly with one of our advisors,
then that person will be noted in our system as your personal contact.
Before
you apply, be sure to view your plan's brochure. They contain
important information regarding benefits, exclusions, limitations,
renewability, and other terms of coverage:
The
Instant Quote page calculates
Preferred Rates. If
you do not think you qualify for Preferred Rates, please contact
us for an exact quote for your situation.
The fastest
and easiest way to get your coverage in force is to Apply
Online,
using the link above. Gather the following information before
you start the application in order to speed the process:
- Your doctors
names, addresses, and phone numbers
- Medications
you currently take or recently took, including dosage and frequency
- Dates of
hospitalization
- The name
of the plan for which you are applying
- Your requested
effective date
- Your credit
card information to cover the initial premium
There
are two other ways to apply for any Time Assurant Health
plan:
- Visit the
secure Time Assurant Apply
Online site to receive an instant quote and then proceed
to Apply Online.
- Apply over
the phone using the Time Tele-App.
- By using
the Tele-App process you may apply with Time and receive an
answer in as quickly as a week.
No payment is required with your application. You
will be billed once you have been approved.
- To qualify
for this accelerated application process, you must first be
able to answer NO to a series of nine health related
questions. View

- If you
were able to answer NO to all nine questions, please view
the Teleapp Application.
When the Adobe Acrobat reader opens with the application,
use the menu bar at the top to save the application to a file
or simply print it out right from the reader. (You may
also email us and
we will send you an application in the mail or by fax.)
- Complete
the application and fax it to us toll
free at 1-866-284-0082.
You may also mail it us at our address below. Once your
application has been received, we will contact you with the
number to call Time for a Personal Health Interview via telephone.
Depending on your medical history, this can take between
10-20 minutes to complete.
- Once
you have completed your interview process, the underwriter
will instruct you as to what comes next. If Time does
not order medical records for your file, you will usually
receive a decision on your application within 7 days.
If medical records are ordered, Time will usually issue a
decision on your application within 7 days of receipt of your
records from your doctor.
- If you were
unable to answer NO to any of the nine questions under
the Teleapp process, you are still eligible to apply for
any plan by filling out the Time application and sending
it in.

- Simply
follow the instructions on the first page to complete the application.
Please make sure to clearly indicate on your cover letter which
plan (One Deductible, RightStart or SaveRight HSA) and which
deductible you are applying for, as the Time application does
not give you any space to list this information.
- Send the
completed application along with first payment to our address
below, or fax it to us toll
free at 866-284-0082.
While you may submit the first month's premium along with
your application, you also may choose to apply on a C.O.D. basis
and have Time bill you for your initial premium after you have
been approved. You may also choose to pay via monthly
bank draft.
Please fill
out the application Completely. Any unanswered
questions will delay the processing. If you have any questions
about filling out the application, you can try to reach one of us
by calling at letting the receptionist know that the call is URGENT.

leave your name and phone number, and someone from our office will
call you back and answer any application questions you have.
Note:
All policies come with a money-back guarantee.
After receiving your policy, you have 10 days to review it.
If for any reason you decide not to keep the policy, you will receive
a full refund of your initial premium. Remember that you also
have access to the guaranteed lowest
life insurance rates available, with easy online quotes and
applications.
