4)  Is the applicant, spouse or any of their dependents, regardless if applying for coverage, currently pregnant or an expectant parent?

Time Insurance does not accept an application on a pregnant female, nor a father of an unborn child.  Coverage for the pregnant mother or the father of the unborn child is not available until the pregnancy has ended.  To be eligible to apply for coverage, the baby must have had a well baby exam at least 2 weeks after medical discharge.

5)  Does any person to be insured have a pending doctor appointment (other than routine) or has diagnostic testing, treatment or surgery been recommended or scheduled that’s not completed?

Ineligible applicants are those with pending non-routine appointments, diagnostic testing, treatment or surgery.  These applicants should postpone applying until after the appointment, treatment or surgery is complete and the results and diagnosis are known.  Wellness services, routine visits or follow-ups to a previously diagnosed illness/disease are eligible to apply.  A newborn must have a well baby exam at least 2 weeks after medical discharge to be eligible for coverage.

6)  Is any person to be insured NOT a U.S. citizen or an immigrant here on visa status, or has any person to be insured been in the U.S. less than one year?

To be considered for coverage, applicants who are not U.S. citizens must:

• Have been in the U.S. for at least one year
• Have a permanent visa or green card status
• Have an established physician* in this country from whom medical records can be obtained

* An established physician provides a complete physical exam which includes age appropriate testing as defined by the   U.S. Preventive Services Task Force.

7)  Has any applicant age 55 or older NOT had a physical within the last 5 years?

Applicants age 55 or older who have not been to a doctor for a complete physical exam in the past 5 years are not eligible for Individual Medical insurance coverage. These applicants will be postponed until after the physical exam has been completed and results known. Visits to a podiatrist, dermatologist and other specialists will not satisfy the complete exam requirement. If the applicant wishes to reapply after completing a physical exam, the following results will need to be provided by and paid for by the applicant:

• Blood Pressure
• Total Blood Cholesterol
• Pap Test (Females)
• Fecal Occult Blood Test
• Mammogram (Females)
• Digital Rectal Exam (Males)

8)  Does any person to be insured have plans for extended travel?

Those contemplating travel (within the next 4 months) or extended travel (3 months or more) are not eligible for Individual Medical insurance coverage. This includes students who travel abroad or study overseas.

9)  Will any proposed insured become eligible for any other form of medical insurance in the next six months?

As a general rule, medical insurance should not be taken if the applicant will become eligible for other medical insurance coverage. Short Term Medical insurance would be recommended in these circumstances.
