straight-up affordable health insurance plans. If youre
19-29 years old, rates can be $64 - $123 per month, depending on
the plan you pick, where you live, your age and your medical history.
This plan is specifically designed for those under age 30, and offers
great coverage for a very good rate. Rates are also very competitive
for older individuals however, please note that the underwriting
is very strict, and they will only take very healthy individuals.

guys have made a good effort to present a plan that is
appealing to younger people. Until now I have been
frustrated by the high rates other insurance companies
charge. The rates for the Tonik Plan from Blue
Cross Blue Shield are very decent, it was very easy to
sign up, and your customer service is great.
If I have the opportunity, I will definitely recommend

plans do not include maternity benefits. Copays for office
visits, ER visits and presciption drugs do not apply towards the
deductible.

Plans are subject to health underwriting. To be considered
for reimbursement, expenses must qualify as covered expenses.
Expenses are also subject to reasonable and customary limits,
unless you use a network, and all other policy provisions, including
determinations of medical necessity.

Coverage
Synopsis:
100% Co-insurance
Tonik Health
Plans are designed for uninsured Colorado adults in their 20s
who are finishing college, starting a career or busy experiencing
life in other ways. The Tonik plan has deductible options
of $5,000 (the Thrill Seeker plan), $3,000 (the Part-Time Daredevil
plan) and $1,500 (the Calculated Risk Taker plan).
After the
deductible, the plan pays 100% of in-network covered expenses,
up to $5,000,000. For doctor visits, there is a co-pay of
$20, $30, or $40 per visit, depending on the deductible chosen.
It also has coverage for vision, dental, and only a $10 co-pay
for generic drugs.
Tonik health
plans include coverage for:
- Physician
office visits, including routine physical exams, covered professional
services, and routine lab work and X-rays done during the office
visit.
- Inpatient
and outpatient hospital and ambulatory surgical center services.
- Ambulance
and emergency room services.
- Preventive
care benefits, including adult preventive exams.
- Generic
prescription drugs from more than 750 pharmacies in Colorado
and over 61,000 pharmacies nationwide.
- Dental
services, including cleanings, exams and X-rays, fillings and
other minor restorative care services.
- Vision
services at $50 per year toward the cost of glasses (or sunglasses),
contacts or routine eye exams.
- An additional
$25 co-payment for standard eyeglass lenses, and a $100 allowance
for frames or an $80 allowance for contact lenses through a
Blue View Vision provider network, as well as out-of-network
benefits.
- Physical
and occupational therapy and/or chiropractic care.
- Tobacco
cessation programs.

Rates:
Rates are
very competitive for those in their 20s, and even for healthy
people in their 40s, 50s, and 60s. Rates
can be found on the
Tonik
rate sheet (pdf - 526KB), or by visiting the Tonik
Online site. Tonik is only available as an individual
plan. It will not cover dependents, so a husband and wife
would have to apply separately.

PPO Network:
Tonik health
plans come with referral-free access to the largest PPO network
in Colorado, with nearly 8,500 health care providers and over
60 hospitals throughout the state. Having access to the
PPO network can mean substantial discounts in what you pay for
your health care, even before you meet your deductible.
The large list of doctors and hospitals can be viewed at the Anthem
PPO Online Provider Directory. Make sure to choose
the "BluePreferred PPO" plan. Policyholders can go to
Blue Cross doctors anywhere in the country, and can even access
a world-wide network.
The BlueCard®
program provides health care access across the U.S. for members
who are traveling and have a medical condition requiring immediate
attention. Members may call the BlueCard number on their
health plan ID card, or visit the BlueCard Doctor and Hospital
Finder at www.bcbs.com. For travel outside of the U.S.,
only medical emergencies are covered.

Underwriting:
Underwriting
on these plans is very rapid, and you will often know the same
day if you have been accepted. Applying for coverage online
is easy, and only takes about 15 minutes to complete. Underwriting
for the Tonik plan is more strict than for other Anthem
Blue Cross Blue Shield plans.
